The proper format for reporting forums

Since we run a variety of domains, forums, and the like, it is important to remember format. Without format, there is nothing.
The Required format for new reports:

Your Name:
If you don't know that, you got problems
Your Credentials:
Do you work for someone? Are you even authorized to file the complaint? If not, go away, we don't want you here
Email:
Never fear, nobody will send spam to you, in fact, they can't harvest the mail address. I've added a bit of security to ensure that. Doubt me? Just take a look at my own signature!
Domain: the domain of the forum
Forum Name: ie forums
The abuse:
Spam/etc
Prove it!:
This is the MOST critical part of the report. You MUST prove that this forum/blog is being abusive. We will not accept complaints without proper proof.

Any complaints or reports not matching those requirements will be thrown out, and we're not joking. If you can't be bothered to fill out a proper complaint , we can't be bothered to investigate the issue. Pretty fair there.
